What Is a Pathway Plan?
A Pathway Plan is a statutory document for young people aged 16+ in or leaving care. It outlines their current needs, goals, and the support they will receive to achieve independence. It should be co-produced with the young person — not written about them.

- Housing: current placement and future move-on options
- Education, Employment & Training (EET): current status and goals
- Health: physical and emotional wellbeing needs
- Life skills: what the young person can do and what support is needed
- Relationships: family contact, support networks
- Financial: benefits, budgeting, financial literacy
